The form below will allow you to request a transcript for:
You DO NOT need to submit this transcript request for the schools listed below because you will need to complete the Self-Reported Student Academic Record (SSAR) in place of submitting a transcript for college admissions purposes. The only time you will send your transcript to any of these schools is AFTER you have accepted admission and will be attending (they will require your final transcript post-graduation):
You also DO NOT need to submit this transcript request if you are applying using the Common Application, the Coalition Application, or SENDedu. Your school counselor will upload your transcript directly through those websites as part of your student report.
- Florida public colleges/universities
- Private or out-of-state colleges/universities
- Scholarships (requiring a full official transcript)

PLEASE KEEP IN MIND...
In-State Transcript Requests:
- After this form is submitted, Paxon sends your request to the Duval County transmission center.
- Duval County sends your transcript electronically to the college.
- The college "picks up" your transcript and will post it on your status screen. This could take up to 3-4 weeks from the date you request it so please be patient if it doesn't show on your status screen immediately.
Out-of-State/Private School Transcript Requests
- After this form is submitted, you will be notified via email to stop back by the school counseling office to pick up the hard copy of the transcript for you to mail.
Dual Enrollment Transcripts: If taking a dual enrollment course, It is up to the student to request their official transcript directly from either Embry Riddle and/or Edward Waters College. Paxon is not able to request this information. The student must contact Embry Riddle and/or Edward Waters College directly and request that they send the official transcript to the college/university for which the student is applying.
